Participants will receive a $50 Visa gift card as a token of appreciation for their time.

To ensure accuracy, the focus group will be recorded by MPHI. Recordings will only be accessible to MPHI staff and will be destroyed after analysis is complete. Throughout the entire process, your contact information will remain confidential and will not be shared.

If you are interested in participating, please review this flyer and complete the registration form.
